Standard Dosing Schedule

Mounjaro is administered once weekly by subcutaneous injection. The standard titration schedule is designed to minimize gastrointestinal side effects while achieving therapeutic efficacy.

Weeks 1-4 0.25 mg once weekly Initiation dose (not effective for glycemic control, used to reduce GI side effects)

Injection Technique

Mounjaro is administered as a subcutaneous injection. Follow these steps for proper administration:

Injection Sites

Rotate injection sites each week to reduce the risk of lipodystrophy (changes in fat tissue).

Administration Steps

  1. Wash hands thoroughly
  2. Check the Mounjaro pen to ensure it contains clear, colorless solution (discard if cloudy or discolored)
  3. Attach a new needle to the pen
  4. Prime the pen (only needed when using a new pen) by dialing to the flow check symbol and pressing the button
  5. Select your dose using the dose selector
  6. Clean the injection site with an alcohol swab
  7. Insert the needle at a 90-degree angle
  8. Press and hold the dose button until the dose counter shows "0"
  9. Keep the needle in the skin for at least 6 seconds after the counter reaches "0"
  10. Remove the needle and dispose of it in a sharps container
  11. Store the pen (without needle attached) in the refrigerator

⚠️ Hypoglycemia Risk

When Mounjaro is used with insulin or insulin secretagogues (like sulfonylureas), the risk of hypoglycemia increases. Your doctor may reduce your insulin or sulfonylurea dose when starting Mounjaro. Monitor blood glucose closely and report symptoms of low blood sugar (shakiness, sweating, confusion, rapid heartbeat).

Special Populations

Renal Impairment

No dose adjustment is needed for patients with mild, moderate, or severe renal impairment. However, Mounjaro has not been studied in patients with end-stage renal disease, so use with caution and monitor closely.

Hepatic Impairment

No dose adjustment is needed for patients with hepatic impairment.

Elderly Patients (≥65 years)

No dose adjustment is needed based solely on age. However, dose selection should be cautious, and patients should be monitored for adverse effects.

Pediatric Use

Mounjaro is not approved for use in patients under 18 years of age. Safety and efficacy have not been established in pediatric populations.
