Mounjaro Dosing Schedule & Administration

Standard Dosing Schedule
Mounjaro is administered once weekly by subcutaneous injection. The standard titration schedule is designed to minimize gastrointestinal side effects while achieving therapeutic efficacy.

Injection Technique
Mounjaro is administered as a subcutaneous injection. Follow these steps for proper administration:
Injection Sites
- Abdomen: Preferred site for most patients. Inject at least 2 inches away from belly button.
- Thigh: Front or side of thigh, middle section.
- Upper arm: Back of upper arm (may require assistance).
Rotate injection sites each week to reduce the risk of lipodystrophy (changes in fat tissue).
Administration Steps
- Wash hands thoroughly
- Check the Mounjaro pen to ensure it contains clear, colorless solution (discard if cloudy or discolored)
- Attach a new needle to the pen
- Prime the pen (only needed when using a new pen) by dialing to the flow check symbol and pressing the button
- Select your dose using the dose selector
- Clean the injection site with an alcohol swab
- Insert the needle at a 90-degree angle
- Press and hold the dose button until the dose counter shows "0"
- Keep the needle in the skin for at least 6 seconds after the counter reaches "0"
- Remove the needle and dispose of it in a sharps container
- Store the pen (without needle attached) in the refrigerator
Missed Dose
If you miss a dose of Mounjaro:
- If ≤5 days have passed: Take the missed dose as soon as possible.
- If >5 days have passed: Skip the missed dose and resume your regular schedule with the next weekly dose.
💡 Tip: Set a Weekly Reminder
Choose a consistent day and time each week (e.g., every Sunday morning). Set a phone reminder or use the Mounjaro app to help you remember your weekly injection.
Dose Adjustments
When to Increase Dose
Your healthcare provider may increase your dose if:
- Your A1C or fasting blood glucose remains above target after at least 4 weeks on current dose
- You are tolerating the current dose without significant side effects
- You have not reached the maximum dose of 2 mg once weekly
When to Delay Dose Increase
Your provider may delay titration if you experience:
- Persistent nausea, vomiting, or diarrhea
- Significant dehydration
- Hypoglycemia (especially if taking insulin or sulfonylureas)
⚠️ Hypoglycemia Risk
When Mounjaro is used with insulin or insulin secretagogues (like sulfonylureas), the risk of hypoglycemia increases. Your doctor may reduce your insulin or sulfonylurea dose when starting Mounjaro. Monitor blood glucose closely and report symptoms of low blood sugar (shakiness, sweating, confusion, rapid heartbeat).
Special Populations
Renal Impairment
No dose adjustment is needed for patients with mild, moderate, or severe renal impairment. However, Mounjaro has not been studied in patients with end-stage renal disease, so use with caution and monitor closely.
Hepatic Impairment
No dose adjustment is needed for patients with hepatic impairment.
Elderly Patients (≥65 years)
No dose adjustment is needed based solely on age. However, dose selection should be cautious, and patients should be monitored for adverse effects.
Pediatric Use
Mounjaro is not approved for use in patients under 18 years of age. Safety and efficacy have not been established in pediatric populations.
Storage & Handling
- Before first use: Store in refrigerator (36°F to 46°F / 2°C to 8°C). Do not freeze. Keep away from the cooling element.
- After first use: May be stored in the refrigerator or at room temperature (59°F to 86°F / 15°C to 30°C) for up to 56 days.
- Keep the pen cap on when not in use to protect from light.
- Discard the pen 56 days after first use, even if there is medication remaining.
- Remove and discard needle after each use. Never store the pen with a needle attached.
