The short answer: Keep Mounjaro in the refrigerator at 36°F to 46°F (2°C to 8°C), in its original carton, and never freeze it. The single-dose pen and single-dose vial can sit unrefrigerated at up to 86°F (30°C) for a total of 21 days. The KwikPen and multi-dose vial get 30 days. When you fly, TSA allows it in carry-on and checked bags, but carry-on keeps it with you and away from lost-luggage delays.
How should you store Mounjaro at home?
The FDA-approved Mounjaro label on DailyMed (revised August 2026) sets the same core rules for every version of the drug:
- Refrigerate at 36°F to 46°F (2°C to 8°C).
- Do not freeze it, and do not use it if it has been frozen.
- Protect it from heat and light. Keep it in the original carton.
The single-dose pen has glass parts. The Instructions for Use say that if you drop it on a hard surface, do not use it. Use a new pen instead.
Which version you have matters. Mounjaro now comes as a single-dose autoinjector pen, a single-dose vial, a multi-dose vial, and the multi-dose KwikPen. Each KwikPen and multi-dose vial holds 4 weekly doses. The storage clock differs between these groups, as the next section shows. How long can Mounjaro stay out of the fridge?
This is where the pen types split. The limits below come from section 16.2 of the prescribing information.
Single-dose pen and single-dose vial:
- Each one can be stored unrefrigerated at temperatures not above 86°F (30°C) for up to a total of 21 days.
- Throw it away after a total of 21 days at room temperature.
The word "total" matters. Time out of the fridge adds up across trips. Putting a pen back in the fridge does not reset the count, so keep a note of the days.
KwikPen and multi-dose vial:
- Unopened and refrigerated, you can use it until the expiration date on the label.
- Unopened but kept at room temperature (up to 86°F), throw it away after 30 days.
- Once opened, you may keep it in the fridge or at room temperature (up to 86°F), in the carton.
- Throw away an opened KwikPen or multi-dose vial after 30 days at room temperature, 30 days after first use, or after 4 weekly doses. Whichever comes first applies, even if medicine is left.
For the KwikPen, the Instructions for Use add one more rule. Remove the needle after each injection and put the cap back on. Do not store the pen with a needle attached, because that can cause leaking, a blocked needle, or air in the pen.
For short trips, the day allowance is rarely the problem. The 86°F ceiling is the harder limit.
What if your Mounjaro froze or got too hot?
The label is direct about freezing. If a pen or vial has been frozen, throw it away and use a new one. Thawing it does not make it usable again. The Instructions for Use also tell you not to inject medicine that is cloudy, discolored, or has particles. Normal Mounjaro is colorless to slightly yellow.
Heat is less clear-cut. The label gives no allowance above 86°F (30°C). It does not say how long a short spike is safe. So if your pen sat in a hot car or checked bag, the label does not support using it with confidence.
In either case, these steps make sense:
- Do not inject a pen you know froze.
- For heat exposure, call your pharmacist or Lilly at 1-800-LillyRx (1-800-545-5979), the number printed on the label.
- Ask your prescriber or insurer about a replacement. Do not try to test the pen yourself.

Can you fly with Mounjaro, and should it go in carry-on or checked bags?
Yes. TSA lists liquid medications as allowed in both carry-on and checked bags. It also lists unused syringes as allowed when they travel with injectable medication. For both, TSA says you must declare them to officers at the checkpoint.
TSA's disabilities and medical conditions page adds the details that matter for a cold-chain drug:
- Medically necessary liquids above 3.4 ounces are allowed in reasonable quantities, but you must declare them.
- Take medically necessary items out of your bag and put them in a bin.
- Ice packs, freezer packs, and gel packs may be frozen, partly frozen, or melted when they keep medically necessary items cool.
- Tell the officer if you do not want your liquid medication X-rayed or opened. Officers then take other steps to clear it.
Carry-on is the better choice. The CDC's guidance on traveling with medicine says to pack medicines in a carry-on in case your luggage is lost. In a checked bag, you cannot control the temperature or get the pen back if the bag goes missing. The CDC also advises:
- Keep medicine in its original labeled container.
- Bring copies of your prescriptions, with generic names (tirzepatide).
- Ask your prescriber for a note, since Mounjaro is an injectable.
- For international trips, check with the embassy of your destination and any layover countries.
If you use ice packs, do not let the pen touch them directly. The label bans freezing, and a pen pressed against a frozen pack can freeze.
How do time zones affect your dose day?
Mounjaro is a once-weekly shot. The label says you can take it at any time of day, with or without food. That gives travelers a lot of room.
The label also allows you to change your dose day, as long as the two doses are at least 3 days (72 hours) apart. It says not to take 2 doses within 3 days of each other. The label does not give separate time-zone rules, so this 72-hour gap is the rule to follow.
A few hours of time-zone shift is small against that 72-hour minimum. If a trip makes your usual day awkward, ask your prescriber about moving it before you leave. Keep injecting in a rotated site, as covered in our injection sites guide.
How do you get rid of used pens and needles on the road?
The disposal rules differ a little by device, per the Instructions for Use in the label:
- Single-dose pen: put the whole used pen in an FDA-cleared sharps container right away. Do not put it in household trash.
- Vials: put used needles and syringes in a sharps container. Always use a new syringe and needle for each injection.
- KwikPen: put used pen needles in a sharps container. After you remove the needle, the used pen body can go in household trash or a sharps container.
If you have no FDA-cleared container, the label allows a heavy-duty plastic household container with a tight, puncture-resistant lid, labeled as hazardous waste.
The FDA's sharps safety page says never to put loose needles in household or public trash, or in recycling bins. It also says never to flush them. For travel, the FDA disposal guide recommends that you always carry a small, travel-size sharps container. It also says to check TSA's website for current rules on sharps when you fly.
Never share a pen or needle, even if you change the needle. The label warns this can spread serious infections.
The bottom line
Store Mounjaro in the fridge at 36°F to 46°F, in its carton, and never frozen. Know your device: single-dose pens and vials get 21 total days at up to 86°F, and KwikPens and multi-dose vials get 30. Fly with it in your carry-on, declare it at security, and pack a travel sharps container. Keep doses at least 72 hours apart if you shift your dose day.
